S.I.D.S. of Pennsylvania has filled more than 6,500 requests for cribs and mattresses for families in Allegheny and Westmoreland Counties.  

Please be advised that vouchers for cribs and mattresses are not issued until the baby is born.  The baby must also be under one year of age.

For your information, the following guidelines must be adhered to in order for a voucher to be sent to the participant:

1.  A written letter from the participant expressing the need for a ‘safe’ sleeping environment for their infant.   The letter should be sent to S.I.D.S. of Pennsylvania, Suite 250 Riverfront Place - 810 River Avenue - Pittsburgh, PA 15212.

2.  The letter must contain the participant’s name, address, telephone number and mother’s date of birth.

3.  Also included with the letter, a copy of the Newborn Identification (containing the footprints of the infant) is required.

Because of the high number of infants in need of a crib and mattress, please make sure that there is truly a need and that all other means of acquiring a crib and mattress have been exhausted before sending us a request.  This program may suspended at any time due to lack of funding.
